Thank you for choosing Koehler Pain Relief for your pain management needs. At KPR, our goal is to provide you with quality medical care to assist you with relieving pain, restoring function and renewing hope
Scheduling
To ensure continuity of care for you as the patient we encourage you to schedule appointments in advance of the follow-up due dates. When calling for an appointment please be prepared to provide your name, telephone number, date of birth, chief complaint/ reason for the appointment, as well as updated insurance information and any changes in contact information.
No Show Policy
In order to provide our patients with quality care on a timely basis, we consider any missed appointment as a no-show. If the appointment is not canceled within two (2) business days advance notice it is a no show. Failure to timely cancel a appointment results in inconvenience to other patients who need access to medical care in a timely manner. Therefore an administrative fee of $50.00 will be billed to your account and payment will be required prior to rescheduling of the appointment.
**Please be advised that no-show charges are not billed to your insurance company.
Payments
Koehler Pain Relief accepts a variety of health insurance plans, Personal Injury Protection insurance, personal checks, MasterCard, Discover, Visa, and American Express. Payments can be accepted in person, over the telephone, through our website on the pay here tab, or via text.
